There are a couple of issues with the printed registration forms: incorrect costs appearing, incorrect father address (see Full Story for details)

Please note that the following issues are known regarding registration forms that were mailed out to returning players:

Registration costs may show incorrectly. Please follow the fee schedule in the Registration Policies and Procedures letter (or the Registration page), as they are correct.

For some Initiation level players, the father's address may appear incorrectly on the registration form. Please be aware that the correct address is in the Hockey Canada system.
