Registration

2013/14 Season Information
Registration packages will not be mailed out to families of currently registered players as we encourage everyone to register using the online registration tool. Installment payments are also available when you register online. Extra registration packages will be made available at all registration sessions.

Registration in person

Registration sessions will be held in the arena lobby on the following dates:

• Tuesday, June 11th from 7:00 pm - 8:30 pm
• Tuesday, August 13th from 7:00 pm - 8:30 pm

Make sure you include the following in your package:

	Completed registration form
	The following cheques made payable to “CPMHA”: Registration amount for the appropriate level (see table below) Raffle ticket cheque: $80 per family
	Copy of Birth Certificate (new players only)
	Copy of one of the following documents to prove proof of residency (new players only): - utility bill - tax bill - lease/rent agreement - bill of sale of home Please note that a copy of your telephone bill is NOT acceptable.
	Player Fair Play
	Parent Fair Play
	Privacy Policy (Personal Information Disclosure Form)

Registration will only be accepted with ALL required fees and documentation. Incomplete registration may result in the player being put on a waiting list.

Registration Priority

Registration will be on a first-come, first-served basis within birth year and division under the following priority:

1. Veteran players (registered with the CPMHA during the 2012-13 season).
2. Former CPMHA players (registered with the CPMHA for a season prior to but not during the 2012-13 season).
3. New players (not previously registered with CPMHA).
4. New and former CPMHA players NOT residing in Carleton Place will be accepted if there is space available and the district has approved your transfer request. This approval must be brought with you to registration.

Age Levels and Fees

NOTE: * Registration amount does NOT include the cost of raffle tickets.

Additional Registration Information

• For parents of children with 3 or more players, the fees for the youngest child will be 50% of the registration amount. 
   
• A $25 charge for all NSF cheques received and must be replaced within 7 days of notification by cash, certified cheque or money order. Failure to do so will result in the risk of player being put on a waiting list.

• Registrar must receive refund requests in writing by December 15^th and no refunds will be given after December 31^st.

• Refunds are pro-rated depending on the number of months attended.

• Full refund minus $80 for raffle tickets will be given to any player withdrawing before September 15^th.

Registration will NOT be processed for players who have not fulfilled their financial obligations with CPMHA or have not returned all CPMHA equipment and jerseys.
