How to report an incident

This form is to be used AFTER speaking to the Head Coach or the Team Manager if the Head Coach is directly involved, when you witness an incident that:

• contravenes the CPMHA code of conduct
• contravenes any CPMHA or HEO bylaws, policies, rules or regulations
• is inappropriate to children, adults, volunteers, parents or officials by CPMHA members or visitors to our games.

This form is NOT the same as, nor does it replace, the Hockey Canada insurance form or injury report.
