TEAM ROSTER: Please upload your team roster, including staff. We understand rosters may not be finalized at the time this form is submitted, so once your roster is confirmed, please email the tournament coordinator with your team. Team rosters must include jersey number, first and last name, and date of birth. TEAM LOGO: we request an image of your team (or association) logo so we may use it on the tournament page, and also for advertising on our social media accounts.
