CPMHA Refund Form
IMPORTANT: Hockey Canada and HEO fees are embedded in all CPMHA registration fees. These fees, which may vary from year to year, are non-refundable. If a refund request is made, it will be processed according to the CPMHA refund policy, less the current Hockey Canada and HEO fees. Players who are expelled or suspended are not entitled to a refund.

CPMHA REFUND POLICY

Refunds for members wishing to withdraw from the current hockey season must be requested by submitting this form. A copy of your request will be sent to the CPMHA Registrar and CPMHA Treasurer to be considered. The date the request is received will be used to determine the applicable refund amount. All refunds are subject to an administration fee, as outlined below:

 Before September 1st: Full refund, less a $65 administration fee.
 September 1st – September 30th: Refund of 75% of the membership fee.
 October 1st – November 30th: Refund of 50% of the membership fee.
 After November 30th: No refunds will be issued for any reason.

Refunds will be processed according to these procedures and timelines. No exceptions will be made, the executive have the right to change this policy in case of an unforeseen circumstance.

Applications will not be processed without this information.

CPMHA will contact the coach to verify all equipment (ex. jersey/goalie equipment) is returned prior to processing any refund.
