    Vulnerable Sector Check Request Form
    It is a priority of the CPMHA to create and maintain a safe environment for all participants in our minor hockey program. An essential and mandatory part of the screening process for our volunteers in the use of the police records check system to identify any volunteer or employee, including those hired under contract, that may cause harm to children or other vulnerable individuals. As per our Constitution By-Law 11, and in accordance with the HEO Policy for Police Records Checks, every person who wishes to volunteer with CPMHA in any capacity is required to have an OPP Police Records Check completed every two years, or sooner, at the discretion of the CPMHA Executive. The completed records check must be submitted to your level Convenor for maintaining. The Police Records Check and the CPMHA Police Records Check Request Letter must be submitted to our local OPP detachment. The Request Letter must be signed by a member of the CPMHA Executive.

    Injury Report Form
    Forms must be filled out in full or form will be returned. This form must be completed for each case where an injury is sustained by a player, spectator or any other person at a sanctioned hockey activity.
