A fee payment schedule is also available:
First payment (50%) is due upon registration
Second payment due 31 July 2025 (34%)
Third payment due 31 August 2025 (33%)
Fourth payment due 30 September 2025 (33%)
Example:
Total price is $995. At registration, 50% is due which is $497.50.
Remaining 50% ($497.50) is broken up into three payments:
Second payment (34%) = $169.15
Third payment (33%) = $164.18
Fourth payment (33%) = $164.18

Trying out for competitive hockey?
If you are trying out for one of the competitive teams: MTK, Silver Seven or Titans, please be sure to choose the “25-26 Competitive Tryout Home association admin fee”
A non-refundable administrative fee of $25 is required to participate in the competitive tryouts.

If you have moved within the CPMHA boundaries (aren't sure? Check here), and your child has played hockey in another association, please proceed with a transfer request based on your personal scenario.
SCENARIO 1: Moves to the CPMHA boundaries.
Players who move to Carleton Place, but who have played with another association, must submit the following:

A completed Hockey Eastern Ontario (HEO) Attestation of Inter-Branch/Association Residency Form (LINK);

Proof of Residency:
i) A copy of the Purchase and Sale Agreement OR Lease Agreement for the new residence (1-year minimum lease), plus
ii) Any two of the following documents:

School Registration
-
Utility, Cable, or Internet Bill (no cell phone), that matches new address

Driver's license (parent) that matches new address

Vehicle insurance/registration that matches new address
SCENARIO 2: Players who live outside the boundaries of CPMHA but within District 4 and wish to play with CPMHA.
Players who wish to play for CPMHA but live outside the boundaries need to complete a House League Transfer Form (Link).
The transfer form approval workflow is as follows:
Receiving Association President Approval
Receiving District Chair approval
Home association Approval
Home District Approval
SCENARIO 3: Players who live within the boundaries of CPMHA and wish to play another Minor Hockey Association.
Players who wish to play for CPMHA but live outside the boundaries need to complete a House League Transfer Form (Link).
The transfer form approval workflow is as follows:
Receiving Association President Approval
Receiving District Chair approval
Home association Approval
Home District Approval
This is a year by year transfer form. The reasoning needs to be justified, and there is no guarantee the transfer will be approved from either association.
