If you have moved within the CPMHA boundaries (aren't sure?
Check here), and your child has played hockey in another association, please proceed with a transfer request based on your personal scenario.
SCENARIO 1: Moves to the CPMHA boundaries
Players who move to Carleton Place, but who have played with another association, must submit the following:
- A completed Hockey Eastern Ontario (HEO) Attestation of Inter-Branch/Association Residency Form (LINK);
- Proof of Residency:
i) A copy of the Purchase and Sale Agreement OR Lease Agreement for the new residence (1-year minimum lease), plus
ii) Any two of the following documents:
- School Registration
- Utility, Cable, or Internet Bill (no cell phone), that matches new address
- Driver's license (parent) that matches new address
- Vehicle insurance/registration that matches new address
SCENARIO 2: Players who live outside the boundaries of CPMHA but within District 4 and wish to play with CPMHA.
Players who wish to play for CPMHA but live outside the boundaries need to complete a House League Transfer Form (
Link).
This is a year by year transfer form. The reasoning needs to be justified, and there is no guarantee the transfer will be approved from either association.
