WHEN YOU NEED TO APPLY FOR A TRAVEL PERMIT
A travel permit is needed for a team traveling to an out-of-branch (i.e. outside of HEO) tournament or exhibition series. See the map of HEO Districts for reference. You can find the application for a Team Travel Permit on the HEO website.

If your team is going to a tournament or exhibition game in the U.S., you must also fill in the USA Hockey / Hockey Canada Team Travel Permit application.

Due to the process required to apply for a travel permit, we strongly encourage you to begin the application process at least one month prior to the tournament.

FILLING OUT THE TRAVEL PERMIT APPLICATION FORM
Hockey ID numbers for both players and team staff can be found on your team's official roster. All other player information can be taken from the official roster. Affiliated players should also be listed if they may be attending the event. Players not listed on the travel permit may not be allowed to play in the tournament.

Official rosters are prepared by the Association Registrar. The Registrar will confirm qualification of team officials, and they may ask you for more information. The Registrar will seek approval for the roster from D4 Registrar. A signed official roster is sent to the Association Registrar who will forward it to the Team Manager. This can take up to two or three weeks.

When all information is completed by the team, the travel permit application (and the Hockey USA/Hockey Canada travel permit application form, if applicable), should be forwarded to your level Convenor for the Association President's signature. The President verifies those listed on the travel permit are the same as those listed on the official team roster, and that any affiliates are acceptable for that team. The President may request the team also forward their official roster at the same time, or have the Association Registrar confirm the names on the travel permit before signing. Once the President signs the form(s), they will send it/them to the District Chair for their signature. The District Chair returns the signed travel permit(s) to the President and/or team. It can take up to two weeks to obtain these signatures.

When filling out the USA Hockey / Hockey Canada form, the Registrar can provide you with the association number. Team ID number can be found on the official team roster (top right hand corner under the barcode). If the tournament organizers assign a team number, that can also be put on the US/Canada travel permit.

Note that U7 teams are only permitted to attend U7 Fund Days within their own district. Prior to participating in a Fun Day, U7 teams must complete a U7 Fund Day Confirmation Form.
