Team bank account authorization request form

When to use this form: Each team is required to open a team bank account. Once this form is completed and signed by an appropriate member of the Executive, it can be taken to the bank to request a CPMHA team account be opened. Note that banks require an in-person appointment attended by both signatories to open an account.

Request letter for a vulnerable sector check (VSC)

When to use this form: Managers are responsible for ensuring that all volunteers who will be interacting directly with players on their team (head coaches, assistant coaches, trainers) have a valid VSC on file with CPMHA. If a VSC will expire at any point during the season, a new VSC must be obtained prior to the first game of the season.

Team officials list

When to use this form: Managers are responsible for ensuring that all coaches and trainers have the qualifications required to volunteer with CPMHA. Use this form to record the names, contact information and qualifications of your on-ice staff, which must be submitted to the CPMHA Registrar by October 15.

Team participants form

When to use this form: Managers are responsible for submitting a list of all players, including their dates of birth, HC numbers and jersey numbers in order to build the official team roster.This form must be submitted to the CPMHA Registrar by October 15.

Incident reporting form

When to use this form: Use this form to report to the CPMHA Executive an incident that contravenes the CPMHA code of conductcontravenes any CPMHA or HEO bylaws, policies, rules or regulations OR that is inappropriate to children, adults, volunteers, parents or officials by CPMHA members or visitors to our games.

Use the form AFTER speaking to the Head Coach or the Team Manager if the Head Coach is directly involved.

Raffle request form

When to use this form: If your team is planning on holding a raffle of any kind, you must request a lottery license on behalf of CPMHA. All raffles MUST be run according to the rules of the Alcohol and Gaming Commission of Ontario (ACGO) and the Town of Carleton Place. The form must be completed no fewer than seven (7) business days prior to the beginning of ticket sales.

Note: “Certify To” refers to the third party that is requesting the proof of insurance (e.g., Town of Carleton Place, City of Ottawa, Sensplex, etc.) not the name of the team or association.
