New Players
This page is for players who have never played in any other association.  If you have played for another Minor Hockey Association (MHA) please go to the transfers page here. 
 
Hockey Canada is strict about playing in your own neighbourhood.  Please check and ensure you are registering with the right association using the HEO Minor Address Lookup Tool: http://maps.hockeyeasternontario.ca/ 
STEP 1: Download and complete the ***NEW PLAYER REQUEST FORM***.  

​This forms needs to be accompanied by the following documents:

• Proof of age (copy of birth certificate)

• Proof of residency 

STEP 2: Email all paperwork to registrar@cpmha.ca

 * Once the above h registrar as been verified a Hockey Canada # will be provided to you.

STEP 3: Once you receive your Hockey Canada # from the registrar you can proceed with online registration.
